A 13-year-old boy from Gloucester, Virginia (VA), was killed in an October 2010 boat accident that occurred underneath James River Bridge. USCG investigator found that the commercial boat operator failed to have a proper lookout or maintain a safe speed, according to the Daily Press. The commercial boat operator has also been charged with involuntary manslaughter.
