What Happened:

A resident at Red Wing Health Center, a for-profit nursing home facility, suffered from dementia and had a history of serious falls at the nursing home facility. The resident had a care plan which specifically mentioned the need for a special wheelchair with a chair alarm and lap belt, to help prevent future falls.

Despite the explicit requirements for the special wheelchair, it was discovered that the nursing home staff placed the resident into a different wheelchair that allowed him to fall onto the floor. Sadly, the resident lost his life after suffering a serious fall.
